If you arrived here and are not the primary account holder/purchaser of the passes, this form will not send your information properly. Please have the primary account holder login or contact the EZpay cancellation number below, to cancel or modify any passes on this account.
- If you are still in a commitment period, your request will be honored immediately after the last payment.
